Elementary Level Admission Checklist

Thank you so very much for your interest in Incarnate Word Academy Elementary Level. All of us at Incarnate Word Academy are very happy to work with your child/children and with you. May God's blessings be with you as you continue to make choices for your children.

The following is required for registration:

• Registration fee: $500 (Please see Family Rate)
• Registration Information Form
• Student Health History Form
• Student Grade / PTO Officers' Form completed
• CCISD Title I Form completed
• Publication Release Form completed
• A certified birth certificate and/or baptismal certificate
• Interview with new parents and student by the principal
• Last report card and objective test results
• Immunizations must be current and filed with the office no later than August 1, 2006 .

We also offer on campus after school care provided by the YMCA.

Please Note:

• When all application requirements have been met, a letter of acceptance will be sent to you, indicating a reserved place for your son/daughter.
• May 1st will be the last day that any refund can be made in the event your child is unable to attend Incarnate Word Academy for the academic year 2006-2007.
