Middle School Admission Checklist

Thank you for considering Incarnate Word Academy Middle School for your child's education. The following items will need to be completed and returned to the Middle School as part of the admission process.

• Registration fee: $500 (Please see Family Rate)
• Registration Information Form
• Student Emergency Form
• Student Health History Form
• Parish Susidy Fee: $100 or Subsidy Form approved and signed by Parish Pastor

           **Total due $600. (If returning the subsidy form signed by your pastor - deduct $100.)

• CCISD Title I Form completed.
• Publication Release Form completed
• Current Immunization Records
• Copy of Birth Certificate
• Current Report Card
• Standardized Test Scores
• Transcript Release Form
• Student Evaluation Request Form (Please submit this form first.)
• Interview with principal; Please call Crissie Suarez at 883-0857 ext. 113 to set your interview.

  _____ Standardized Test Scores

• An evaluation test in Math is required for all new students. This is an evaluation test only. This will not be considered a grade. Please contact Crissie Suarez for test date information.

Please Note:

• When all application requirements have been met, a letter of acceptance will be sent to you, indicating a reserved place for your son/daughter.
• May 1st will be the last day that any refund can be made in the event your child is unable to attend Incarnate Word Academy for the academic year 2006-2007.
