High School Registration Checklist

Thank you for considering Incarnate Word Academy High School for your child's education. The following items need to be completed and submitted as a complete packet:

• Registration fee: $550 (Please see Family Rate)
• Registration Information Form
• Student Emergency Form
• Student Health History Form
• Parish Subsidy Fee: $100 or Subsidy Form approved and signed by Parish Pastor

           **Total due $650. (If returning the subsidy form signed by your pastor - deduct $100.)

• CCISD Title I Form
• Publication Release Form
• Current Immunization Records
• Copy of Birth Certificate
• Schedule appointment with Counselor to complete Course Selection Form. (Contact MaryLou Loidl, Academic Counselor)
• Current transcript, last report card, and most recent test scores. (Fax to: MaryLou Loidl, Academic Counselor)
• Student's conduct, attitude and grades must be acceptable.
• Schedule interview with principal (Contact Sandra Trevino, Administrative Assistant to Principal)

Please Note:

• When all application requirements have been met, a letter of acceptance will be sent to you, indicating a reserved place for your son/daughter. The acceptance letter will be mailed by June 30, 2006.
• May 1st will be the last day that any refund can be made in the event your child is unable to attend Incarnate Word Academy for the academic year 2006-2007.
